In relation to the employee
- Copy of Work and Social Security Card
· Copy of Employee Registration Form
· If you are a PJ employee – copy of your company registration number
· Copy of personal identification document
Occupation Health Certificate valid considering all the risks of the activity – Mandatory to access the controlled area
· Signed safety (NR1) covering all activity risks – Mandatory to access the controlled area
· Signed Basic NR10 Certificate – Mandatory to access the controlled area
· Signed NR10SEP certificate – Mandatory to access the controlled area
· PPE form – Mandatory to access the controlled area
· NR11 certificate – if the employee performs work involving moving loads (hoist, crane, PTA)
· NR12 certificate – if the employee performs activities with machines and equipment
· NR35 certificate – if the employee performs work above 2 meters
· Authorization for activities with electricity (NR10)
· Authorization for activities at height (NR35) – in case of activities above 2 meters
Regarding the company:
· Risk Management Program
· PCMSO
· Emergency Plan or Emergency Contact
· If using a crane, presentation of the Rigging Plan and ART of the Technical Manager is mandatory.
To access the facilities (controlled areas) and begin activities, company employees must:
· Respect COTESA's safety protocol for preventing COVID-19 (Wear a protective mask and periodically sanitize your hands with 70% alcohol; Access to people with COVID-19 symptoms is prohibited)
· Access is prohibited for people with pacemakers;
· Use mandatory safety equipment (PPE): (Use Class II Flame Retardant Clothing, Electrician Safety Footwear and Class B Helmet with chinstrap);
· Sign the Warning Form for Third Parties when accessing the substation.
